Client Needs Category, Safe and Effective Care Environment, includes two
concepts:
Coordinated
Care accounts for
18-24 percent of questions on the NCLEX-PN® exam. The nursing actions

Assignments, Client Rights, Concepts of Management and Supervision,
Confidentiality, Continuity of Care, Establishing Priorities, Ethical Practice,
Informed Consent, Legal Responsibilities, Quality Assurance, Referral Process,
and Resource Management.
Safety and
Infection Control accounts for 10-16 percent
of exam questions. Nursing actions include Accident/Error Prevention, Handling
Hazardous Materials, Home Safety, Injury Prevention, Emergency Response Plan,
Medical and Surgical Asepsis, Incident Reporting, Safe Use of Equipment,
Security Plan, Standard/Transmission-Based/and other Precautions, and Use of
Restraints and Safety Devices.
The second
Client Needs Category is Health Promotion and Maintenance. These questions
account for 6-12 percent of the exam. Nursing actions tested include the Aging
Process, Ante/Intra/Postpartum and Newborn Care, Data Collection Techniques,
Developmental Stages and Transitions, Disease Prevention, Immunizations, High
Risk Behaviors, Lifestyle Choices, Self Care, and Health Promotion Programs.
The third
Client Needs Category is Psychosocial Integrity. It accounts for 9-15 percent
of the exam and tested nursing actions include Coping Mechanisms, Grief and
Loss, Mental Health Concepts, Spiritual Influence on Health, Sensory/Perceptual
Alterations, Stress Management, Support Systems, Therapeutic Communication,
Chemical Dependency, Behavioral Management, Crisis Intervention, Coping
Mechanisms, and End of Life Care.
The final
Client Needs Category is Physiological Integrity. It includes four concepts:
Basic Care and Comfort accounts for 7-13 percent
of questions on the NCLEX-PN® exam. The nursing actions included in this
subcategory are Assistive Devices, Elimination, Mobility, Non-pharmacological Comfort
Interventions, Nutrition and Oral Hydration, Personal Hygiene, as well as Rest
and Sleep.
Pharmacological
and Parenteral Therapies accounts for 10-16 percent
of the exam. Tested nursing actions include Adverse Effects, Expected Effects, Medication
Administration, Pharmacological Pain Management, and Side Effects.
Reduction of
Risk Potential accounts for 9-15 percent
of the exam. Its tested nursing actions include Diagnostic Tests, Laboratory
Values, Potential for Body System Alterations, Potential for Complications of
Diagnostic Tests, Treatments, and Procedures, Therapeutic Procedures, and Vital
Signs.
Physiological
Adaptation accounts for
7-13 percent of the exam. Its tested nursing actions include Alterations in
Body Systems, Basic Pathophysiology, Fluid and Electrolyte Imbalances, Medical
Emergencies, and Unexpected Response to Therapies.
Everyone answers a minimum
of 85 questions to a maximum of 205 questions. Regardless
of how many you answer, you will be given 25 experimental questions that do not
count for or against you. The exam administrators use them to test for future
questions on the exam.
The NCLEX is scored using dichotomous
scoring, so you can either pass or fail the exam. Currently, to pass the
NCLEX-RN, the standard is 0.00 logits–or answer questions correctly at least
50% of the time
Passing and Failing The decision as to
whether a candidate passes or fails the NCLEX is governed by three scenarios.
Scenario #1: The 95%
Confidence Interval Rule This scenario is the most common for NCLEX candidates.
The computer will stop administering items when it is 95% certain that the
candidate’s ability is either clearly above or clearly below the passing
standard.
Scenario #2:
Maximum-Length Exam Some candidates’ ability levels will be very close to the
passing standard. When this is the case, the computer continues to administer
items until the maximum number of items is reached. At this point, the computer
disregards the 95% confidence interval rule and considers only the final
ability estimate. ? If the final ability estimate is at
or above the passing standard, the candidate passes. ? If the final ability estimate is below the passing standard, the
candidate fails.
Scenario #3:
Run-Out-of-Time Rule (R.O.O.T.) If a candidate runs out of time before reaching
the maximum number of items and the computer has not determined with 95%
certainty whether the candidate has passed or failed, alternate criteria are
used. ? If the candidate has not answered the
minimum number of required items, the candidate automatically fails. ? If at least the minimum number of required items were answered,
then the final ability estimate will be based on all responses given before the
exam time expired. If the score is at or above the passing standard, the
candidate will pass; otherwise, the candidate will fail.
